Please note, this form is designed to gather more information about you and the procedures you’re interested in. You’re able to REQUEST a specific date for a consultation however that is NOT a guarantee of the date and time you submit here. Someone from our office will contact you soon to schedule and confirm your free consultation.

Please note that all consultations are free EXCEPT for Vein Consultations, those are billed through your insurance and a co-pay may be due at the time of visit. If you do not have insurance, and wish to pay cash for the consultation fee, it is $150.00 due at the time of visit.

To help you better plan your schedule, consultations with Dr. Palmer are only Monday – Thursday from 9AM-5PM (Closed 12-1pm for lunch). Currently we do have consultations on Fridays or on weekends. Our last appointment time would be no later than 4:00pm. We try to have at least one consultation day set aside each week OR every two weeks, depending on our surgical schedule.
